Why this matters

This form lets VA speak with *Thank You For Your Service (TYFYS) about your benefits information—so we can help without you making every call. It is not* appointing legal representation; it’s permission to share info.

*Time:* 5–10 minutes (online)

*You’ll need:* VA.gov sign-in; TYFYS contact info to enter on the form (we’ll provide the exact phone/email in your TYFYS portal task).

### Option A — Submit online (recommended)

1. *Open the online 21-0845.* Type https://www.va.gov/supporting-forms-for-claims/ and press *Enter. Scroll to “Authorize VA to release your information to a third-party source (VA Form 21-0845)” and click Submit an authorization online*.

2. *Sign in. Use Login.gov or *ID.me**.

3. *Fill in TYFYS details. For Third-party individual/organization, enter Thank You For Your Service (TYFYS)* and the TYFYS phone/email from your task.

4. *Choose what VA can share. Check the boxes for benefits information* we’ll need to assist you.

5. *Submit and save proof. Complete the signature step, then download or screenshot the submission confirmation*. Save as TYFYS_210845_Submission_[YYYY-MM-DD].pdf.

6. *Upload to TYFYS. Open your TYFYS task “VA Authorization (21-0845)”* and upload the confirmation.

### Option B — Download, sign and send

7. *Download 21-0845 (PDF). Fill it out and hand-sign*.

8. *Submit to VA.*

Fastest electronic upload: QuickSubmit* (VA’s current online Evidence Intake tool) → Type https://eauth.va.gov/accessva/?cspSelectFor=quicksubmit in your browser, sign in and upload the PDF.

9. *Upload to TYFYS.* Scan your submitted form or upload the QuickSubmit confirmation to your TYFYS task.
